Individuals that are represented as a member of a group/band must legibly print/type the name of the group or band in the first blank space provided.  All members must sign in the appropriate space at the end of the contract.  The compensation for all members of a band will be disbursed to the first representative of the group/band as indicated.  Talent represented by VDMPMM must be at least 18 years old and own a personal United States Social Security Card.  Talent that is not at least 18 years old must submit a written letter of consent from a legal parent or guardian.  Please Note:  print/type your name legibly (individual/group/band) or you will be disqualified from representation hereby.  (Print this page using your browser or control/p function).

 

I/We,                                                     , hereby agree to allow VDMPMM to distribute information and products bearing my/our image, craft or talent in accordance with corporate distribution laws of the United States of America (USA) and International Political and Free Trade Countries to the extent of the USA cooperative alliance agreements.  VDMPMM agrees to submit all future compensation received for hereby talent, less 5% United States Dollars (USD), to hereby talent within 30 days of the clearance of funds into USD currency.  VDMPMM agrees not to misrepresent/misconstrue or otherwise alter hereby talent images/products/media to be unrecognizable or destructive to the professional reputation/image of hereby talent.  Representation hereby is limited to National Commercial Prime Time.

 

VDMPMM reserves the right to disassociate with any talent engaged in any illegal or illicit acts.  VDMPMM reserves the right to disassociate with any talent that will or has engaged in any illegal or illicit act, or otherwise agrees to engage in behavior that is adult oriented, pornographic, or visually or audibly offensive in either legal or illegal form.  VDMPMM is an English/Christian organization and reserves the right to disassociate with talent associated with illegal religious or terrorist organizations/individuals/states, or engaged in obviously physical/psychological destructive behaviour/activities such as illegal/legal substance abuse, promotion/distribution/sale of illegal substances/products, or the sale/distribution/promotion of the satanic occult or related antisocial/terrorist religions/organizations/activities.

 

Talent represented by VDMPMM is hereby prohibited from solicited/unsolicited use or promotion/distribution of the name or association of VDMPMM or any of its legal subsidiaries/associates/affiliates or represented talent or corporate/employed affiliates in any respect, or activities that are not directly associated with the express promotion of the hereby talent according to the terms indicated and outlined herein this TALENT REPRESENTATION CONTRACT as agreed to by signature below.  Solicited or unsolicited or illegal use or abuse or misrepresentation or promotion of the name of VDMPMM or any associated or affiliated corporations/talent/products or representation for personal or professional or otherwise benefit than according to this TALENT REPRESENTATION CONTRACT is strictly prohibited and will result in prosecution to the fullest extent of the law.   VDMPMM reserves the right to the final decision of media distributed in cooperation with talent hereby.

 

Talent hereby must sign and return this TALENT REPRESENTATION CONTRACT and the following identification and contact information required and written in English only.  Please Note: Groups/Bands must submit the following for each individual signed below or attached: 

 

1.       Notarized Copy of International Passport or Valid United States Government or International Government Identification.

2.       Notarized Copy of Valid and Current United States Social Security Card.

3.       Notarized Copy of All Professional Union Memberhips/Affiliations.

4.       Notarized Copy of Proof of Legal Residence Address, and Viable Telephone Number and all Work/Current Contact Information.

5.       All of the Above for both Talent and Legal Parent/Guardian along with a Notarized Letter of Consent (if not 18 years of age).

6.       Professional Business and Talent Resumes from the age of 18 years old (or from your first job if not 18 years of age).

7.       All Professional Media, including Videos, Photographs, CD’s, DVD’s, Products, Brochures, Composites or otherwise indication of talent or products for VDMPMM to represent.  PLEASE DO NOT SUBMIT PORNOGRAPHY OR PROFANITY. 

 

VDMPMM reserves the right to disassociate with talent engaged in any unprofessional activities whatsoever.  VDMPMM does not/will not guarantee hereby any proceeds for the submission of any products or media or talent or performances.  All media submitted hereby will become the property of VDMPMM immediately upon receipt and will not be returned to the talent.  VDMPMM does not/will not guarantee representation to any talent unless by otherwise written contract, and may discontinue hereby representation at any time for any reason.  This TALENT REPRESENTATION CONTRACT and associated services of VDMPMM is/are completely non-exclusive to hereby talent, and does not/will not disallow/bar this talent from any employment or contract/agreement to employment or distribution of any products or media of proposed or hereby represented talent.  VDMPMM will represent hereby talent using legal/professional methods of promotion and distribution.  This TALENT REPRESENTATION CONTRACT may not be misconstrued to exact any personal or otherwise auxiliary relationship considerations for any talent or professional associates of VDMPMM not expressly provided by hereby stipulations.
